Self Referral:

Mental Health Treatment Providers Referrals:

• With a complete referral packet and an engaging guardian, we can schedule an appointment within 60 days.

  • Referral packet can be found on the Forms link.​

• Send the referral packet through email to Amie.Merritt@renewedjourneyresources.hush.com ; or fax it to 855-718-2713

• Once all of the required information is provided, the referral will be processed and an intake will be conducted. 

• Once the evaluation is completed, it will take 8 weeks for a report to be generated and submitted for approval (depending on case presentation or other delays outside of RJPS control).

If you have a secondary insurance, this would take over as a primary insurance and if Renewed Journey is not contracted with private insurance, client is responsible for service fees. We will need to be provided a copy of the insurance card, please inform guardians of this.

• Banner Health, Steward Health Choice (Prior Authorization Needed), Mercy Care Plan, and United Health (Prior Authorization Needed).
